• Staff Nurse Proficiency Certificate Level, AHW, and ANM for the relevant course
• Certificate of Educational Qualification
• Certificate of Citizenship
• Certificate of Expiry Certificate registered with the concerned Council
• copies of other required documents.

Examination fee: Rs.600 for Staff Nurse, and Rs.500 for AHW and ANM.

Salary Scale: The agreement between the two parties will be as per the agreement.

Service facility: will be as per contract agreement.
